Lightbulb VoIP/IPT Dial out or Dial in

In the Enterprise Composite Network Model, where does it suggest the breakout points or PSTN connections reside for Voice use????

In Diane Teare's Ciscopress book the only reference to PSTN is within the VPN/Remote Access Module as part of the Enterprise Edge however this need is said to only be for termination of dialin and VPN sessions over the PSTN.

What do you guys/gals think?
